Management of extensively drug-resistant Acinetobacter baumannii bacteremia with cefiderocol and sulbactam-durlobactam: a case report

Summary

A 56-year-old male with a history of hypertension, diabetes, and end-stage renal disease presented with extensively drug-resistant (XDR) Acinetobacter baumannii bacteremia, diagnosed via blood cultures. The patient developed septic shock, requiring vasopressors and intubation. Initial antibiotic regimens were ineffective due to resistance. Treatment was escalated to a combination of cefiderocol and sulbactam-durlobactam, resulting in clinical improvement and pathogen clearance. This case highlights the successful use of novel antibiotic combinations for challenging XDR A. baumannii infections in a critically ill patient, demonstrating a potential therapeutic strategy.

Key note: Combination therapy with cefiderocol and sulbactam-durlobactam can be an effective treatment for XDR Acinetobacter baumannii bacteremia.
